Who is responsible?
Kliniek voor Tandheelkunde Wassenaar is the data controller for the details collected via this website. The practice is located at Van der Oudermeulenlaan 1, 2243 CR Wassenaar. The remaining practice details (telephone number and e-mail address) will be listed here as soon as they are available.
What details do we collect?
Via the registration form we only collect the details you enter yourself: your name, e-mail address and telephone number, and optionally your area of interest, your preferred day and time, how you found us and a remark. This website does not place tracking cookies and does not build visitor profiles.
What do we use your details for?
Solely for your registration as a new patient: confirming your registration, planning your first appointment and keeping you informed about the opening. The legal basis is your consent, which you give by sending the form. Your details are not used for other marketing purposes and are not sold or shared with third parties, with the exception of our hosting provider, which technically operates the website and e-mail.
How long do we keep your details?
Until your registration as a patient has been completed, or until you let us know you no longer wish to receive updates. After that, your details are deleted. Once you are registered as a patient, your details become part of your patient record, to which the statutory (healthcare) retention periods apply.
Your rights
You have the right to access, correct or delete your details, and to withdraw your consent at any time. Please contact us via the form; after the opening this can also be done by telephone or e-mail. You also have the right to lodge a complaint with the Dutch Data Protection Authority (Autoriteit Persoonsgegevens).
Security
The website is served over a secure connection (HTTPS). Form details are delivered to the practice by secure e-mail and are not visible on the website.
